Assumption of risk
Dance is a physical activity that involves risk of injury. By signing below I acknowledge that I (or my child, if a minor) am participating voluntarily and assume all risk of injury arising from participation in Knox Kpop classes, rehearsals, or performances.
Release of liability
I release Knox Kpop, Asami Martin, instructors, volunteers, and venue operators (including Karns Community Club Youth Center) from any claim resulting from such injury, except where caused by gross negligence.
Photo / video
Knox Kpop may capture photos and video at classes and events for studio records, promotional materials, and social media. Use the checkbox below to opt in or out of social media use specifically. Studio archival use is always permitted.
